中国女性生殖健康促进公益项目倡议书

近年来,中国人口随着计划生育国策的长期贯彻落实发生了结构性的变化。2013年底,中共十八届三中全会通过审议,“在坚持计划生育基本国策的同时,放开单独二孩,逐步调整完善生育政策”。这是继《国家人口发展“十二五”规划》提出“应完善人口政策,促进人口长期均衡发展”以来的重大的计生政策调整。这也给女性生殖健康带来了新的机会与挑战。
目前我国女性生殖健康仍面临诸多风险,随着中国女性的社会和经济地位逐步提升,整体社会环境也更加开放,女性对于自我发展有了更高的要求,她们对待婚育的态度正发生着改变,年轻人初次性行为时间提前,婚育时间推迟成为显著的趋势。但与此同时因避孕方式选择不当造成的流产及重复流产量问题也十分严重。我国每年仍有大量女性接受人工流产手术,这给女性健康带来严重伤害。为改善这一现状,近年来,我国计生工作者在中国妇女发展基金会的倡导和推动下,积极推行“流产后关爱”项目,至今已惠及百万女性。在此基础上,还应加强预防工作,从根本上解决因避孕方式选择不当而引起的人工流产以及因此给女性带来的生殖健康隐患。为此《中国妇女发展纲要》提出应“保障妇女享有避孕节育知情选择权,减少非意愿妊娠,降低人工流产率”,“提高妇女生殖健康服务水平”,同时“推行避孕节育知情选择,提供避孕节育优质服务。加大避孕知识宣传力度,提高妇女自我保护意识和选择科学合理避孕方式的能力”。
为更有效的开展和落实工作,应以女性在生殖健康方面新的需求为出发点。基于女性婚育周期的变化,她们需要了解更多的生殖健康及科学避孕知识,并能够根据自身需求,调整避孕计划,做出适合自己婚育状况的避孕选择。为满足这些需求,我国现有的计生服务应在内容及形式上都应进行优化:如提供方便、易获得的、权威专业的避孕信息渠道,进一步加强避孕知识教育,促进女性的避孕知情选择权;提升医院、药店等育龄女性计生服务接触点的专业化服务水平;增加对年轻未婚人群的主动计生宣教和专业服务等。
鉴于此,我们需要因循世界卫生组织提出的《关于性与生殖健康服务宣传教育推动的策略方法》,联合政策研究、科学技术、公共宣教以及相应的服务提供机构,合力共同支持“中国女性生殖健康促进项目”,以期通过优化计生服务内容,提升计划生育服务专业服务水平,并通过提高宣传与教育的效果,实现中国女性人人享有避孕知情权及生健康优质服务,特别是在以下几个方面,进行中国女性生殖健康促进的探索:
1、              在PAC项目基础上上继续完善相关医患教育,并在医院、药店等女性避孕及生殖健康服务接触点,对医生、药师等计生服务提供者进行标准化的计生与避孕选择咨询与服务流程的建设及培训,提升计生服务提供者的专业水平;
2、              在高校开展健康避孕观念及知识教育,提升年轻人的避孕知识水平;
3、              建设权威、专业、有公信力的新媒体平台,为年轻人及育龄女性提供更易于获得的、易于接受的咨询与服务;
4、              借助世界避孕日等公益宣传机会,开展针对全部育龄人群的生殖健康及避孕知识宣传教育,并推动全社会对这一议题的关注和支持。
 
女性生殖健康不仅关系着个人、家庭的幸福,对整体人口素质提升、社会经济的稳定发展有着重要意义。因应计生政策调整的新形势,为满足中国女性在生殖健康方面多元化的需求,应进一步完善我国计生服务体系,提升专业服务水平,通过广泛的宣传与教育使女性切实获得避孕选择知情权,使育龄女性能够做出科学的、适合个人的计划生育与避孕选择。我们携手同心,倡议全社会力量关注中国女性生殖健康,并为其献计献策、共同努力!

In recent years, the Chinese population has had a structural change with the long-term implementation of the national policy of family planning. At the end of 2013, the Third Plenary Session of the 18th Central Committee of the Communist passed the discussion, Open the two-child policy for Chinese couples with one spouse being an only child,while insisting on the basic family planning policy, so as to improve the childbearing policy. This has been a major adjustment of the family planning policy since Improving the population policy to promote long-term balanced development of the population put forward by <The 12th Five-Year Plan for National Population Development>. It also brings new opportunities and challenges for womens reproductive health.
Currently, women’s reproductive health in China still faces many risks. With improvement of the social and economic status of Chinese women, the entire social environment has also been more open. Women have higher requirements for their own development, and their attitude towards marriage and child-rearing is also changing. It is a significant trend that young people have sex for the first time earlier and the time of getting married and having children is put off. At the same time, the problem of abortion and repeated abortion resulted from improper of contraception is serious. Every yeara large number of women have induced abortions in China. This seriously harms women’s health. To improve the situation, on the initiative and promotion of the CWDF, China family-planning workers have been carrying out the Post-Abortion Care program in recent years, and one million women have benefited from it. Prevention should also be strengthened to fundamentally solve the reproductive health risks caused by improper induced abortion resulted from improper contraception. Therefore, <Outline for Women's Development >of China proposes that “we should guarantee women’s informed choice of contraception, reduce unwanted pregnancy and the induced abortion rate, improve the service level of reproductive health”, “promote informed choice and provide quality contraceptive service, increase the publicity of contraceptive knowledge, improve women’s self-protection awareness and ability to choose a scientific and reasonable contraception. “
To carry out the work more efficiently, we should start from the new needs of women. They need to learn more about reproductive health and scientific contraception based on the change of the cycle of marriage and child-rearing, adjust the contraception plan based on their own needs and make suitable contraceptive options. To meet these needs, the current family planning service in China should be optimized in contents and forms, e.g. providing an accessible and authoritative information channel, strengthening education of contraceptive knowledge, promoting womens informed choice, improving the professional service level of hospitals, drug stores and other places where women get contraceptive and reproductive health service, increasing active education and professional service for young singles etc.
In view of this, we need to jointly support the Chinese Womens Reproductive Health Promotion Project by following the Strategy of Promoting Education of Sex and Reproductive Health Service of WHO and joining hands with organizations of policy research, science, public education and relevant service providers, so as to improve the level of professional service by optimizing the content of family planning service, and realize the goal that every Chinese woman has the right to know and has quality health service by improving education effects. We will especially explore the promotion in the following aspects:
5、Continue to improve doctor-patient education on the basis of the PAC program, and provide building and training on standardized consultation and service flow for doctors and pharmacists, and improve the professional level of service providers;
6、Provide education about healthy contraception in colleges and universities to improve the knowledge of young people;
7、Establish an authoritative, professional and accredited new media platform to provide accessible consultation and service for young people and women of childbearing age;
8、Carry out education on reproductive health and contraception for all people of reproductive age, and drive the entire societys concern and support to the topic by making use of the publicity of World Contraception Day
Womens reproductive health not only concerns the happiness of individuals and families, but is of important significance to improvement of population quality and stable development of social economy. Based on the adjustment of the family planning policy, we should improve the system of family planning service and the professional service level to meet the diversified needs in reproductive health, and let women get the right to know for contraceptive options through wide publicity and education, so that women of childbearing age can make scientific and informed choices. We join hands to propose that the entire society pay attention to Chinese womens reproductive health and contribute suggestions and efforts to it!
